Administrative

Administrator
Oversee the day-to-day operations of a rehabilitation and health care center focused on short-term rehab and long-term care. Ensure excellent customer satisfaction and service as well as employee morale. Must have knowledge of state and federal regulations. Oversee and ensure regulatory compliance. Previous experience in long-term care administration and nursing home license required. BA/BS necessary.

Executive Director
Ability to direct the operations of state-of-the-art assisted living communities. Responsible for ensuring residents are completely satisfied with the programs, services and environment; building strong employee, customer and community relations; and developing and following financial and marketing plans. Candidates should possess strong communication and interpersonal skills, the ability to lead and inspire, management experience and marketing and sales skills. Long-term care/assisted living experience needed as well as a commitment to seniors. BA/BS required.

Nursing

Director of Nursing/Assistant Director of Nursing
Oversee and manage nursing personnel to maintain quality standards of care and advises medical staff, department heads and Administrator regarding nursing. Establish nursing goals and objectives to meet patient and resident needs. Long-term care or acute health care experience preferred. Nursing supervisory experience needed. Current RN license in Illinois required.

MDS Coordinator
Oversee resident assessments and track Medicare/Medicaid case-mix documentation to ensure compliance with guidelines and managed care case management policies. Work with interdisciplinary team to ensure appropriate patient care plan tracking. A nursing license is preferred.

Registered Nurse
Provide nursing care to residents and patients and supervise CNAs in their daily activities. Must be licensed in Illinois or eligible to take the licensure exam. Experience preferred. CPR required. NEW GRADS ARE WELCOME TO APPLY.

LPN
Full-time and part-time LPNs for all shifts. Assess residents and patients clinical conditions, administer medications and treatment plans. Supervise the unit and CNAs. Must be licensed in Illinois or eligible to take the licensure exam. LTC experience preferred.

CNA
Full-time and part-time CNAs for all shifts. Work with RNs, assist with monitoring vital signs and patient transport. Previous experience helpful. CNA certification required.

Dietary

Food Service Manager/Director
Oversee kitchen operations and menus, including the preparation and delivery of meals. Recruit, hire, train and supervise food service staff. Responsible for ordering food, food service supplies and equipment and tracking food service expenses. Adhere to food service sanitation regulations.

Host/Hostess
Greet rehabilitation patients, residents and their guests when they choose to enjoy their meals in the dining room. Identify appropriate seating for all diners. Strong interpersonal skills a plus.

Server
Greet rehabilitation patients and residents who choose to enjoy their meals in the dining room. Set tables, take food orders and bus tables. Ability to understand menu items and answer questions. Must adhere to all sanitation and food-handling protocols. Strong interpersonal skills a must.

Dietary Aide
Assist in preparing, serving and cleaning up after meal services.

Cook
Operate the kitchen as well as ensure the timely preparation and delivery of quality food for rehabilitation patients and residents. Knowledge of safe food handling principles and the ability to operate kitchen equipment a must.

Dietitian
Assesses the nutritional needs of patients and residents to develop appropriate care and meal plans. RD license required.

Ancillary

Personnel Director
Responsible for all facets of Human Resources, including recruiting, benefit administration, employee relations and workers comp. Orientate new employees to the facility. Process payroll in an efficient and confidential manner. Maintain up-to-date personnel files. The ideal candidate will have strong interpersonal communication skills and experience in long-term care. Knowledge of computers helpful. BA/BS required

Social Service Director
Provide assistance to residents and family members throughout the assessment and care planning process as well as transitional and discharge planning activities. Excellent communication skills required. College degree in social services or related field and MSW/LSW/LCSW desired. BA/BS required.

Activity Assistant/Director
Identify and schedule recreational activities to address the individualized needs and interests of rehabilitation patients and residents. Previous experience working with older adults and in leading recreational activities, arts/craft, intellectual/educational activities, community outings desired. BA/BS required.

Maintenance Technician
Oversee electrical and mechanical systems in the facility. Must be able to perform frequent lifting, moving and/or carrying. Previous experience required.

Housekeeper
Responsible for the overall cleanliness of the facility. Ensure areas are free from safety hazards.

Receptionist
Acts as a liaison between the facility and its patients, residents and guests. Greet and assist guests with requests, ensuring their needs are met. Excellent customer service skills a must.

Marketing/Patient Liaison
Promote facilities and their programs and services to prospective patients, residents, family members as well as hospital social workers and discharge planners and physicians. Identify the needs and goals of potential customers. Communicate with patients, residents and families regarding placement services and help to identify appropriate levels of care. Meet census goals. LTC experience necessary.
